Can anyone tell me, if it is possible to update Vista Ultimate to 7 Ultimate without starting from scratch?

Yes, you can upgrade but a clean install is usually better.

If you have your data on a separate partition or disk to the OS (recommended) then a clean install is relatively painless and appears to take around 30 min for Win 7.

Start here http://www.microsoft.com/windows/windows-7/get/upgrade-advisor.aspx
Download and run the W7 Advisor program so you know what problems there might be.
